Ed Knittel's Form-O-Matic™ - a product of tastypopsicle.com

The Form-O-Matic is a free/open-source JavaScript based form generator for all* of your form making needs. It is XHTML Strict and 100% accessible (and so is the form it creates for you). It does use inline styles (mostly just for text field widths) because I like the flexibility of being able to have consistent form element sizes across all platforms. Should you choose not to use it then that's cool.

I was inspired to create this after looking at the Accessible Form Builder at Accessify.com and looking for a generic form generator online but finding nothing of use. Because I need to make a lot of forms I needed an easy way to quickly mock something up - with and without the use of tables. This is 3 days worth of effort prime for the picking.

Should you like the style that I use to make my forms you will probably want to download the CSS file as well. [formomatic.css]

*Note: obviously it cannot possibly meet all of YOUR needs but it does meet most mine.
